  Hey Rick, I checked the website you mentioned it seem they are using some sort of custom code. I looked at the source code and could not find any reference to a plugin. Your best bet would to use https://codecanyon.net/item/modal-survey-wordpress-poll-survey-quiz-plugin/6533863?ref=WPMultiverse and hire a developer to style it how you like.
